We believe that every individual deserves to have their end-of-life wishes respected and honoured.
Our Advanced Care Planning services are designed to help patients and their families make informed decisions about their care preferences, including medical treatments, pain management, and quality of life goals.
Our caring and skilled professionals work closely with patients and families to provide guidance, support, and resources for creating personalised care plans that reflect their unique values, beliefs, and desires.
We understand that Advanced Care Planning can be a sensitive and emotional process, and we are here to listen, validate, and assist with empathy and understanding.
Advanced Care Planning can be done at anytime by anyone.

We recognize that patients and families facing advanced illness need a supportive community that understands their unique challenges and provides comfort and companionship.
We have partnered with the Compassionate Communities program which aims to create a nurturing environment where patients and families can connect with others who have similar experiences, share their thoughts and feelings, and find solace in knowing they are not alone.
We strive to create a warm and welcoming community that promotes healing, comfort, and emotional well-being to those who need it.
This program is currently in it’s inception and we see huge potential for it to grow. But we need YOU to turn this into a reality. We are calling on community volunteers who can commit to a few hours a fortnight to give back to their neighbours who may be doing it tough.

We believe that open and honest conversations about death and dying are crucial for promoting understanding, acceptance, and healing.
Our Death Cafe’s will provide a safe and supportive space for patients, families, and community members to discuss their thoughts, fears, and questions about death in a non-judgmental and respectful environment.
Led by our facilitators, these informal gatherings encourage participants to share their stories, experiences, and insights about end-of-life care, grief, and bereavement. We aim to normalize the conversation about death, reduce stigma, and empower individuals to navigate the complexities of the dying process with dignity and grace.
Keep an eye on our socials where we will announce our upcoming Death Cafe meet-up’s.
